ACA Tax Form Services

Employers are responsible for providing employees with proof of health insurance coverage and filing the appropriate forms with the IRS and state agencies.
We support both small and large groups to ensure full compliance with Affordable Care Act (ACA) requirements.






1094/1095-B Services (Small Group)

For small groups (generally under 50 full-time equivalent employees), we handle all aspects of “B Form” ACA reporting.
This is ideal for self-funded or level-funded employers.

MA 1099-HC (Massachusetts Residents)

The 1099-HC is a Massachusetts state tax form proving health insurance coverage. Each MA resident with eligible coverage receives this form from their carrier.

You can download a copy by logging into your medical insurance carrier’s portal, or by calling the member services number on your ID card.






State Reporting & Typical Timeline

We manage filing deadlines and reporting requirements on your behalf. Below is a typical schedule for the 2024 tax season:

  • Q3 & Q4 2024: Collect employer-level information and implementation forms
  • Late Dec 2024 – Early Jan 2025: Collect census and enrollment data
  • Late Jan – Early Feb 2025: Finalize filings for review and sign-off
  • Mar 2: IRS employee form distribution deadline
  • Mar 24: E-file all groups (after last-minute corrections)
  • Mar 30: Notify groups of successful e-filing; request corrections if IRS flags issues
  • Mar 31: IRS e-filing deadline
  • Apr 30: Submit any final corrections
  • Year-round: Support with back filings and responses to IRS penalty letters




Penalties for Non-Compliance

I.R.C. §4980H(a) – “Sledgehammer”

Failure to offer Minimum Essential Coverage (MEC) to at least 95% of full-time employees.
Penalty up to $2,970 per employee per year.

I.R.C. §4980H(b) – “Tack Hammer”

Offering coverage that is unaffordable or fails Minimum Value (MV). Penalty up to $4,460 per affected employee per year.

IRS §§6721/6722

Failure to file correct forms, furnish correct employee copies, or e-file when required can result in penalties of $310 per form.
After August 1, 2025, this increases to $630 per form.







Frequently Asked Questions







Form 1095-B shows you had health insurance that met Minimum Essential Coverage. Since 2019, you don’t need it for federal taxes,
but it may still be useful for certain state filings or exchange coverage verification.



The MA 1099-HC contains similar information to 1095-B, but it’s specifically for Massachusetts state tax purposes.
You’ll receive it from your insurance carrier (not the GIC). Massachusetts’ individual mandate and penalties are separate from federal rules.



The GIC reports 1095-B coverage for non-Medicare retirees and survivors. Medicare retirees’ coverage is reported by the
Centers for Medicare & Medicaid. Medicare plan members do not receive an MA 1099-HC.



  • 1095-C: Employer-offered coverage information for Applicable Large Employers (50+ FTEs).
  • 1095-A: Marketplace coverage and Premium Tax Credit details (issued by the exchange).
